Today we are running a test of a possible change to our appointment system.

You will be asked to complete an online form giving our GPs more detail than you would normally give when booking an appointment.

Please complete the form & send it back as soon as possible.

This will be reviewed by a GP, & then we will let you know what will happen next.

You can do this now, rather than wait on the ‘phone, by choosing ‘contact us online’ from the home page & then choose ‘medical request’.

If you do not have a smartphone or access to a computer, reception will ask you for the information our doctors need to know.

Rest assured we will respond to all requests today, so please do not be put off by the fact that we won’t be giving you an appointment immediately.
